Treat moving targets and spare healthy tissue with certainty using Elekta Unity Comprehensive Motion Management

With unparalleled visualization and precision Elekta Unity has pushed the boundaries of what is possible in Radiation Therapy. The team at the Advanced Radiation Oncology Department at Negrar, Italy has been treating patients using MR-guided Radiation Therapy since 2019. Recently their Elekta Unity was upgraded with Comprehensive Motion Management (CMM) with True Tracking and Automatic gating to deliver real-time adaptive treatment in the presence of motion.

One of the first patients treated using CMM was a patient with a liver metastases of stage IVB high-grade serous carcinoma of tubular-ovarian type. The treatment was an SBRT fractionation delivering 50Gy in 5 fractions with a PTV margin 5 mm isotropic and 10 mm inferior.

During treatment delivery the 3D position of the target was tracked continuously, and the beam was gated when the target moved out of the defined boundaries. During the whole treatment the patient was able to breathe freely, without the need to perform difficult-to-tolerate breath holds. Total on-table time was on average 36 min.

Patient was treated while breathing freely using automatic gating in exhale position.

Conclusion

The team found Unity more favorable compared to conventional treatment for this patient as it provided more certainty that the dose was delivered to the target whilst avoiding the normal tissue.
