Elekta Studio's development was deeply influenced by our customers' invaluable input and collaboration.

Marjolein reminds us that having everything in one room was introduced long ago with real-time prostate treatment. She says it wasn’t named Elekta Studio at the time. However, with the integration of the ImagingRing, dedicated table, applicators, afterloaders, and planning system, she recalls that Elekta Studio emerged as a comprehensive solution that brings all elements together in a single room.

Marjolein shares: “ Elekta Studio's introduction has changed how we engage with customers. Brachytherapy Setup Simplified to Increase Patient Comfort and Improve Accuracy

Marjolein Idzes, Director of Marketing Strategy at Elekta, compares the differences between a traditional brachytherapy workflow and a one-room workflow with in-room imaging.

The traditional brachytherapy setup often involves patient transportation to different locations within the hospital, causing discomfort and potential changes in staff interaction. With Elekta Studio's vision of consolidating all components in one room, these challenges can be overcome. Patient movements are minimized by having all equipment, including the afterloaders and ImagingRing, in the same room. This means the patient can be imaged and treated in the same position, reducing the risk of applicator displacement. Additionally, the consistency of staff presence throughout the procedure enhances patient comfort and facilitates focused care.

Elekta Studio's integration of various imaging modalities, such as 2D, X-ray, 3D, and fluoroscopy, allows physicians to utilize the most suitable imaging modality at any given moment. Fluoroscopy aids needle navigation during insertion, while cone beam 3D data provides a comprehensive patient set for contouring and treatment planning in the Oncentra planning system. By minimizing patient movement and avoiding the need for multiple imaging devices, the chance of applicator or needle displacement is significantly reduced, improving the overall accuracy of the brachytherapy procedure.

The benefits of Elekta Studio extend beyond patient comfort and accuracy. With the ImagingRing and its large bore, data can be acquired in the exact treatment position, further decreasing applicator displacement. Moreover, immediate adjustments, such as needle repositioning, can be made during the procedure without requiring time-consuming patient transportation.

The team showcased their experience with Elekta Studio at the 2023 American Brachytherapy Society conference, highlighting the groundbreaking Cone Beam ImagingRing system. They shared their journey of implementing ImagingRing from vault design to clinical validation with their first patient. They highlighted the workflow benefits and improved accuracy that brings to brachytherapy planning.

An end-to-end study to assess the ImagingRing in treating HDR GYN patients

The radiation oncology team conducted a study to assess ImagingRing as an effective way to improve patient safety and workflow efficiency within the time constraints of treating HDR GYN patients.

The study aimed to investigate the image quality and workflow for GYN brachytherapy using ImagingRing. The study utilized the CIRS GYN Phantom, the Venezia CT/MR applicator, and end-to-end testing to determine appropriate clinical workflow and quality assurance procedures. An abstract of this study was presented at the 2023 ABS conference, and you can find it here.

Results

A Venezia ring and tandem applicator, with four interstitial needles, were implanted in the GYN phantom. 3D volumetric images were acquired using CBCT and conventional CT. Both image sets were reconstructed using metal artifact reduction algorithms. Anatomical landmarks were identified in CBCT images and fused to CT using rigid registration in MIM.

Registration of CBCT images to CT demonstrated an agreeable distal dwell position (1-mm), providing reasonable verification of applicator placement. CBCT image quality was determined to be appropriate for brachytherapy planning. Applicator models were employed in Oncentra and catheter reconstruction of needles, and a simulated HDR treatment fraction was created for both CT and CBCT image sets, demonstrating ImagingRing to be an appropriate tool for brachytherapy treatment planning.

Conclusion

End-to-end testing showed that given the versatility and complexity of the Venezia applicator, ImagingRing provides a valuable layer of accuracy for QA implementation in a clinical brachytherapy setting.

Adding ImagingRing to the brachytherapy suite improves patient safety, comfort, and efficiency of the HDR procedure, as there is no need to transfer the patient multiple times.

"During our patient image registration test using the GYN phantom and Zephyr tabletop (CT/MR/CBCT), we aimed to assess the registration accuracy while considering motion and transfer between different imaging modalities such as CT, MR, and Cone beam CT. The in-phantom results were promising as we achieved an accuracy within one millimeter. This level of precision allows us to confidently register applicators and assess their alignment across various imaging techniques." - Dr. Mourtada.

A study that investigates a quantitative comparison between conventional CT and CBCT.

In transitioning from their CT-based workflow (CT-Simulation) to Elekta Studio with ImagingRing, the Jefferson team evaluated the inter-observer variability of contouring organs-at-risk (OARs) for cervical cancer HDR brachytherapy. Results from the first patient are presented here.

Clinical case

A stage IIB cervical cancer patient was implanted with the CT/MR Venezia Tandem (6-cm, 30-deg) with 26-mm diameter Lunar Ovoids. Images were acquired with ImagingRing CBCT and CT-Sim.

Method

  • The bladder, rectum, sigmoid, and lower bowel were independently contoured by eleven radiation oncologists on Ring-CBCT and CT-Sim in Oncentra TPS (Remote access to Oncentra)
  • An HDR plan was created on the CT-Sim and Ring CBCT images. 600 cGy was prescribed to CTV-HR. Both plans had the same dwell positions and weights.

The study assessed inter- and intra-observer variability in OARs contouring between ImagingRing CBCT and CT-Sim images.

The study also analyzed the dosimetric differences between ImagingRing CBCT and CT-Sim plans for the highest dose (0.1cc, 1cc, and 2cc) received by the bladder, rectum, small bowel, and sigmoid.

Results

The results showed no statistically significant differences in the highest bladder, rectum, and small bowel dose between ImagingRing CBCT and CT-Sim (p>0.05). However, there was a statistically significant difference in the sigmoid high dose (p < 0.001), although the difference was not clinically significant.

Specifically, the D2cc (the dose received by 2cc of the volume) for the sigmoid was 23.3% for ImagingRing CBCT and 31.6% for CT-Sim, respectively.

Conclusion

These findings indicate minimal dosimetric differences between ImagingRing CBCT and CT-Sim for most organs. It provides valuable insights for accurately planning and delivering brachytherapy treatments with ImagingRing, contributing to optimal radiation dose delivery.

How does ImagingRing in the Brachytherapy vault improve treatment efficiency?

Elekta Studio: an all-in-one room comprehensive system to enhance brachytherapy workflow and improve the quality of care.
Elekta Studio is a suite of products designed for every step of the brachytherapy process, including applicator and needle insertion/guidance, treatment planning, verification, and treatment delivery. One component of the solution, the ImagingRing, is a novel mobile cone beam computed tomography (CBCT) imaging device. Physicians can perform the whole treatment procedure in one room by bringing imaging to the patient instead of taking the patient to imaging. In-room imaging ensures a shorter overall brachytherapy procedure while providing reduced applicator or needle displacement and improved patient comfort.

ImagingRing in HDR Vaults: Workflow and room organization
The team explained that the ImagingRing must be commissioned before treating the first patient. The brachytherapy team received user training to adapt their workflow.

  • Ensuring the correct orientation and positioning of the ImagingRing will prevent interference with the team's movement around the patient and ImagingRing during the procedure. Due to the ImagingRing's mobility, you must pay attention to the power and network cables.
  • Patient orientation with the room entrance must be considered to ensure patient privacy. Same for anesthesia and the OR light. Marking a "default" position of the ImagingRing on the floor will assist with setting the device up consistently.
  • A key consideration is the setup and space for the radiation oncologist during applicator insertion. Having the 2D and 3D imaging capabilities available during insertion provides the physician additional insight. The only issue is the need to leave the vault and close the door to acquire images.
